1. We will do a background check on every new employee. Any misdemeanors or felonies will be reviewed before a hiring decision can be made.
2. Must have current auto insurance and agree to increase your liability limits to 100,000-300,000-100,000, and provide a Certificate of Insurance with Legacy stated as an interested party. This must be done and turned into the office before a decision of employment will be made.
3. Must have and pass a physical examination.
4. You will receive a mileage reimbursement of .36 a mile
5. Must have a TB skin test or proof of test within a year.
6. Valid State Drivers license.
7. Must have a cell phone. Employees will receive a cell phone allowance based on your job description.
8. Must have a checking or savings account as payroll is direct deposited.
9. Social Security Card/Proof to work legally in state.
10. Must have a copy of certification/license.
11. CNA must be able to pass written test and complete training.
12. If you are an hourly employee, you will be paid overtime after physically working more than 40 hours in a week.
13. Registered Nurses are hired on a salary basis and are reimbursed with Flex time after physically working 80 hours, including "on-call rotation"

15. After an employee has completed their 90 day probationary period, they are awarded (5) five PDO days. After a year of service the employee is awarded (3) three weeks of PDO days and after five years of service (5) five weeks of PDO. Any scheduled time off must be prearranged and approved by supervisor.
