Administrative Nurse or Patient Care Coordinator

JOB SUMMARY:
Assess, plans, coordinates, implements, and evaluates the nursing care provided to terminally ill patients who need palliative nursing care. Coordinates interdisciplinary team. Ensures comfort needs of patients are met appropriately utilizing established standards of nursing hospice care, and Hospice approved policies and procedures.
SUPERVISION:
• Supervised by: Clinical Director
• Supervises: Registered Nurses/LPN's and Home Care Aides/Homemakers, Office Managers, Social Workers, Chaplains, Volunteer/Bereavement Coordinators
• Works closely with:
Quality Assurance, all Hospice staff members
JOB SPECIFICATIONS:
Education: Required: RN. Desired: Bachelor of Science in Nursing, Certification in Gerontology, Community Health, or Hospice Nursing.
Experience At least one year of supervisory experience. Required: One year of nursing experience in hospice and/or home health care. Desired: Three years experience in supervision of, and/or management in home health, hospice or palliative nursing care.
Licenses, certifications, and/or registrations:
a. Current State Registered Nursing License
b. Current Driver's License
c. Proof of current automobile liability insurance
d. Certificate of Liability with limits of 100/300/100
• Specialized knowledge and skills
a. Patho-physiology related to common terminal illnesses and related medical and surgical treatments.
b. Ability to work with terminally ill patients and their families
c. Nursing care relevant to pain and symptom control
d. Humanitarian attitude and commitment to Hospice philosophy
e. Ability to cope with families experiencing emotional stress
JOB FUNCTIONS:
• Coordinates, conducts and maintains accurate records of all clinical in-house meetings, which include patient care conferences, weekly staff meeting of clinical staff, in-service education programs and submits original documentation.
• Marketing and community education activities, including but not limited to seeing 4-6 doctors a week, educating nurse discharge planners, participating in health fairs and civic groups.
• Reviews and approves schedules for staff providing direct patient care.
• Responsible for developing and maintaining hospice programs. Takes an active role in training.
• Appoints hospice staff members when and where appropriate.
• Attends Interdisciplinary Group meeting and conducts staff meetings.
• Acts in a liaison capacity with other agencies and professional groups on a local and state level.
• Implements a quality assurance mechanism to assess the overall hospice program.
• Implements personnel policies. Verifies that there is an alternate designated to act on behalf of an employee when absent.
• Provides administrative direction.
• Develops and reviews the hospice's goals and objectives.
• Program development and coordination for non-nursing clinical services.
• Provides supervision to staff.
• Assists in developing contracts with other institutions.
• Remains current on reimbursement and program rules and regulations, industry trends and development of hospice concepts and procedures.
• Ensures patient and personnel confidentiality.
• Explores new service needs of the community.
• Makes home visits as needed.
• Participates in care planning.
• Review/audit patient charts to ensure documentation is done appropriately according to regulations and Legacy's policies.
• Assists with continuing education and training of existing and new staff.
• Informs family of their options in making decisions that are right for their family.
• Promotes community awareness of hospice.
• Performs other related duties as assigned.
PHYSICAL REQUIREMENTS:
• Must be able to perform limited to full range heavy work requiring sitting standing, walking, and driving.
• Must possess vision, hearing and physical dexterity necessary to maintain a driver's license and operate an automobile.
• Must be able to turn, lift, carry and otherwise transport patients.
• Must be able to climb stairs, access and operate elevators, and access patients' homes that may not have conventional access
• Must possess vision, hearing, feeling, physical strength and dexterity necessary to perform all the duties of a Hospice Patient Care Coordinator.
• Must be able to communicate effectively both verbally and in writing. Must be able to give and receive instructions and information, both in person and via telephone.
• Work requires exerting force of up to 100 pounds occasionally, and/or up to 50 pounds frequently and/or up to 20 pounds constantly in moving objects or materials.
